SUMMARY STATEMENT:

This is an RN position that will be responsible for the day-to-day clinical efficiency and effectiveness of the Prevention Clinic Program. Your role will be pivotal in enhancing patient care through education, quality improvement champion for individuals undergoing treatment, program initiatives and collaboration. Duties include but are not limited to patient assessment, implementation, and evaluation related to discharge education and assistance in treatment planning for self-management as well as follow-up care to reinforce adherence with the treatment plan.

MAJOR AREAS OF ACCOUNTABILITY:

  • Review and assign all new patient consults for prevention clinic, based on diagnosis of patient. This may involve gathering information about symptoms, medical history, and current health status from outside medical facilities.
  • Manage prevention providers pool/in-basket results per in-basket management policy. Provide patients with test results, assess, and evaluate patients understanding, and refer to physician if necessary.
  • Authorize prescription refills as directed by supervising physician, per MHVI prescription refill policy.  Telephone/fax prescriptions to pharmacy.
  • Demonstrate ability to work with confidential and controlled information including patient records, physician DEA numbers and prescriptions.
  • Facilitate effective communication between different healthcare providers to ensure that all team members are well informed about patient progress and any changes in treatment plans.
  • Provide ongoing education and training to staff about the latest treatment options and care strategies for lipid disorders which helps maintain high standards of care.
  • Pre visit planning to include verifying all orders, diagnostic and or lab tests are completed before patient visit to maximize efficiency.
  • Responding to patient questions and health needs per triage guidelines, gathering information to establish adequate assessments and forwarding to appropriate provider as needed.
  • Ongoing work with CV quality department to ensure department is meeting regulatory and quality guidelines and goals.
  • Accurate and thorough documentation of the triage process is maintained to ensure continuity of care and to provide a clear record for other health care providers.
  • Assist in the clinic as requested especially with comprehensive discharge orders.
  • Competent in performing all LPN/MA duties.
  • Collaboration with all MHVI departments regarding updates and changes in the prevention clinic.
  • Point person for and manage pharmaceutical representatives and lunch lectures.
  • Management of complex medication prior authorizations and or patient assistance programs.
  • Coordination of monthly and quarterly department meetings per provider request.
  • Strong adaptability skills.
  • Promote the team approach – lead by example.
  • Ability to communicate respectfully and effectively with physicians, patients, staff and public.
  • Other duties as assigned by leadership.

  1. Current RN licensure with a BSN preferred.
  2. Strong organizational, multitasking and time-management skills
  3. Excellent communication and interpersonal skills for patients and team interactions
  4. Knowledge of prevention services preferred
  5. Ability to work in a face-paced environment while maintaining attention to detail
